All-on-4 or Full Arch?
Where the All-on-4 name comes from, how the number of implants is decided, what each of our four pathways covers and what a fixed arch costs in Greenwich in 2026.
How Do All-on-4 and Full-Arch Implants Differ, and How Much Do They Cost in Greenwich?
"Full-arch implants" is the general description: one fixed bridge replacing every tooth in a jaw. All-on-4 is a trademarked version of that idea which uses four implants, with the back two tilted. At Peninsula Dentists on Greenwich Peninsula, a fixed arch comes through one of four pathways, priced arch by arch: Healed Foundation £9,300, Staged Treatment £11,500, Same-Day Fixed Teeth £13,500 and Premium Guided Zirconia £14,950. The first two use four implants; Same-Day and Premium use four to six, at the same fee whichever number your bone needs. The CT scan, 3D planning and a written plan come with every pathway.
Where the All-on-4 Name Comes From
All-on-4 is a trademarked treatment concept, not a type of implant. It describes a complete arch of teeth carried on four implants, with the back two tilted so their tips reach firm bone further forward. The tilt steers clear of the nerve in the lower jaw and the sinus above the upper back teeth, and often removes the need for a bone graft. Developed in the 1990s, the approach is now used by implant teams worldwide under all sorts of names.
Because the brand caught on, people now apply it to almost any fixed teeth on implants, hence the confusion.
What "Full-Arch Implants" Covers
Full-arch implants is the umbrella term: every tooth in one jaw replaced by a single bridge fixed to implants. Small screws hold it, so it stays in day and night and only your dentist takes it off. All-on-4 falls inside that definition, and so does a five- or six-implant bridge.
Our implants are MegaGen AnyRidge, which carry the CleanImplant Trusted Quality seal. On Healed Foundation and Staged Treatment an arch takes four. On the Same-Day and Premium pathways it takes four to six, and there the count is not a premium tier; it follows from your bone. Dense lower jaws often need only four, while softer upper bone may do better with the load spread wider. Dr Imran Riaz sets the number from your CT scan, and on those two pathways your fee stays the same whatever it turns out to be.
Know Your Number Before You Decide
A free consultation and CT scan show which pathway suits you and how many implants your jaw needs, and you leave with a fixed written fee.
Four, Five or Six Implants?
All three layouts work well, and on our Same-Day and Premium pathways any of them can be used. What changes is how the biting force is shared out.
- Four make a broad base because the back pair are angled. This is the standard layout, right for most lower jaws and many upper ones.
- Five are a sensible halfway point when the scan suggests four would be borderline.
- Six divide the load between more anchor points. We lean towards six for soft upper bone, a powerful bite or clenching, or where a spare implant adds security if one ever runs into trouble.
A photo or a quick look cannot give a reliable number. A CT scan measures the height, width and density of your bone, and that settles it. If a clinic names a number before scanning you, ask why.
What All-on-4 Costs in Greenwich
The fee is set by your pathway: where your mouth is starting from, and what it needs on the way to fixed teeth.
| Pathway | Per arch | 12 months at 0% |
|---|---|---|
| 01 Healed Foundation: a healed jaw with no teeth, 4 implants, acrylic / PMMA bridge | £9,300 | £775 a month |
| Zirconia on a titanium bar instead, Healed Foundation only | +£2,500 | |
| 02 Staged Treatment: teeth out first, implants after healing, acrylic / PMMA bridge | £11,500 | £959 a month |
| 03 Same-Day Fixed Teeth: extractions plus 4 to 6 implants at one sitting, acrylic / PMMA bridge | £13,500 | £1,125 a month |
| 04 Premium Guided Zirconia: as Same-Day, plus a custom surgical guide and zirconia over titanium | £14,950 | £1,246 a month |
These are guide prices; your own fee is fixed in a written plan before anything starts.2 The monthly column assumes Chrysalis Finance at 0% over 12 months,4 available on treatment from £1,500. Six months interest free, or longer terms at 14.9% APR representative, are also offered. Both jaws are two arch fees, shown together as one total on your plan, and you can treat one now and the other later.
For context, practices around Greenwich and SE London we checked in October 2026 publish fixed full-arch fees from about £9,000 to £14,000 per arch, and Greater London price guides quote £12,000 to £18,000. Check what a figure covers: a low headline price may be for a removable denture rather than fixed teeth, and quotes differ on whether extractions, grafting, same-day teeth and the final bridge are in the price. Our implant cost guide lists every implant fee we charge.
What Each Pathway Includes
- Consultation and CT scan. The consultation is free, and the 3D scan, digital records and implant planning are part of every pathway.
- A written treatment plan with your fixed per-arch fee.
- Extractions and routine grafting where needed, on Staged, Same-Day and Premium. Healed Foundation is for a jaw with no teeth left to remove.
- The implants: Healed Foundation and Staged use four; Same-Day and Premium take between four and six.
- Teeth while you heal: your own denture on Healed Foundation, a removable temporary denture on Staged, and fixed temporary teeth on surgery day where the bone allows on Same-Day and Premium.1
- A custom surgical guide from advanced digital planning, made with MegaGen R2, on Premium only.
- The final bridge: acrylic / PMMA on the first three pathways; zirconia on a custom-milled titanium bar on Premium, or on Healed Foundation for +£2,500.
- Reviews until your treatment is complete, and a five-year guarantee for non-smokers who keep up three-monthly hygiene visits.
IV sedation with our oral surgeon, Dr Imran Riaz, is £500 per session, and most full-arch patients choose it. More extensive bone augmentation, sinus procedures and long-term maintenance are quoted separately if you need them.
Choosing the Bridge Material
The implants underneath are identical whichever bridge you have. The bridge is what you see, bite with and clean, so it shapes daily life most.
- Acrylic / PMMA, used on every pathway except Premium, is the lightest and least costly. A chipped tooth is simple to mend, but acrylic wears and can stain over the years, so expect to renew the teeth at some point.
- Zirconia over a titanium bar, part of Premium or added to Healed Foundation for £2,500, sets extremely strong, stain-resistant ceramic teeth over a milled metal bar for rigidity, which suits long spans and heavy bites.
Our guide to full-arch bridge materials compares them in more depth.
The Timeline, Step by Step
- Consultation and CT scan. One visit with Dr Riaz.
- Planning. Implant positions are fixed on the scan and your pathway confirmed. On Premium the custom surgical guide is 3D printed, and on Same-Day and Premium the temporary bridge can be made in advance.
- Surgery day. On Same-Day and Premium, remaining teeth are removed, the implants go in and, where the bone allows, the temporary bridge is fitted before you leave.1 On Staged Treatment, extractions happen first and implants wait until you have healed, and Healed Foundation patients go home in their existing denture. Most people have IV sedation.
- Healing. Roughly three months of softer food while the implants fuse with the bone.
- The final bridge. Designed from a digital scan once the implants are firm.
- Maintenance. Hygiene every three months, which keeps the guarantee valid.
Reviews are short, and North Greenwich is five minutes from Canary Wharf on the Jubilee line. Our teeth in a day guide covers what the same-day bridge can and can't do.
A Fixed Arch or an Implant-Retained Denture?
Where budget is the obstacle to fixed teeth, there is a step in between. An implant-retained denture snaps onto two lower or four upper implants. It holds firm for eating and speaking, yet you remove it to clean, so it is still a denture rather than fixed teeth. Fees start at £8,500 a jaw. Our full-arch versus dentures guide sets the three choices side by side.
What to Ask Any Clinic
Wherever you go, these questions make quotes comparable:
- How many implants will I have, and what decided that?
- Which implant system, and how long is its published record?
- Will you take a CT scan and use a surgical guide?
- Which clinician places the implants, and which lab builds the bridge?
- Are extractions, grafting, temporary teeth, the final bridge and reviews all covered?
- Which material is the final bridge?
- What guarantee is there, and on what conditions?
The written plan we give you answers each of these before you commit.
