Signs Worth Getting Checked

Pain, tenderness or swelling around the implant
Gum that bleeds, looks red or has receded from the crown
A bad taste, or discharge from the gum beside the implant
A crown that moves, or an implant that feels loose
A chipped or cracked crown
Food packing between the implant crown and the next tooth
Changes in the bone on an X-ray, which you can't feel but we can see

Loose, or Urgent?

A crown or a screw that has worked loose is one of the commonest implant problems, and it is often fixable. An implant that itself feels loose needs to be seen promptly. In pain? Call before 9.30am to be seen the same day, on 020 8788 6688.

If swelling is spreading, you have a fever or you find it hard to swallow, call NHS 111 straight away, day or night. Call 999 if the swelling is affecting your breathing.

Common Causes

Most implants last for many years. When one gives trouble, there is usually a clear reason.

Peri-Implantitis

Infection of the gum and bone around an implant, the implant version of gum disease. Caught early, it often settles with thorough cleaning; left alone, the bone around the implant shrinks.

A Loose or Broken Screw

The small screw that holds the crown onto the implant can loosen or snap. The implant underneath is often sound.

A Worn or Cracked Crown or Abutment

Crowns can chip, and the abutment, the connector between implant and crown, can crack, especially under heavy biting.

An Implant That Didn't Bond

Now and then the bone doesn't fuse with the implant in its first weeks or months. It usually shows as soreness or looseness before the final crown goes on.

Bite Overload or Grinding

Too much force, from grinding, clenching or a bite that sits slightly high, can loosen parts and strain the bone. A night guard helps protect it.

An Implant in a Poor Position

An implant placed at an awkward angle or depth can be hard to keep clean or to fit a good crown to, and sometimes has to be replaced.

What Happens at Your Consultation

The consultation is free, wherever your implant was placed. Bring anything you have, especially from treatment abroad.

1

Your History and Records

When and where the implant was placed, and what has changed. Records, X-rays, scans, an implant passport or card, or simply the implant brand all help.

2

Examination, and X-Rays If Needed

Dr Riaz examines the implant, the gum around it and how your teeth meet, and takes X-rays if they're needed at the time.

3

A CT Scan, If It Would Help

A CT scan (£120) shows the bone around the implant in 3D, and whether the two are still fused, which a standard X-ray can't always show.

4

Identifying the Implant

We try to identify the implant system from your records, X-rays or implant passport. Parts for some systems are hard to get, so sometimes the crown, or the implant itself, has to be replaced rather than repaired. If that is the case, we tell you plainly.

5

Your Options, in Writing

You leave knowing the options, what each involves, and a written fee. There is no obligation to go ahead.

Your Options

Tightening or replacing a screw or crown. Often the simplest fix, as long as the implant underneath is sound and parts for it can be found.

Treating peri-implantitis. Professional cleaning around the implant, sometimes with minor gum surgery, then hygiene visits every three months to keep it settled.

Replacing a failed implant. The implant is removed, the site heals for a few months, with a bone graft if it needs one, and a new implant is placed in the healed bone. IV sedation is available.

Problems across a whole arch. Where several implants or a full-arch bridge are failing, our full-arch pathways start from £9,300 per arch.

Your Guarantee

Our five-year guarantee for non-smokers who keep up three-monthly hygiene visits covers implants we place. It can't cover work done at another practice or abroad. If a new implant here is the right answer for you, we go through the guarantee it would carry before you decide.

Implants Placed Abroad

We assess implants placed abroad in the same way as any other, with no judgement. Bring any records, X-rays or scans you were given, and the implant brand or passport if you have it, because repairs depend on knowing the exact system. Our guides to why dental implants fail and to dental treatment abroad go into more detail.

Questions

Replacing a failed implant, work done abroad, and what our guarantee covers.

Can a failed implant be replaced, and how long does it take?–
Usually, yes. The failed implant is removed, the site is cleaned and often grafted, and after a few months of healing a new implant is placed in the healed bone, so allow several months from start to finish. A new implant and crown is from £2,250, with a bone graft from £700 if one is needed, and Dr Riaz confirms the plan and fee in writing after your scan.
